Paid Time Off (PTO)
Provides employees with flexible paid time off from work that can used for such needs as vacation, personal or family illness, doctor appointments, and other activities of the employee’s choice. Full-time employees begin accruing PTO hours beginning on their employment date and may begin using PTO at the beginning of the first full pay period following their initial 90 days of employment. PTO is based on length of service and hours worked plus PTO hours paid in accordance with the following schedule:
 
*Part-time employees will be eligible for 40 hours of PTO per year under the following conditions:
a)    Continuously employed as a part-time or PRN employee for a full calendar year; and
b)    Worked a minimum of 1,560 hours in the prior calendar year as a part-time or PRN employee.

401 (a) Defined Contribution Retirement Plan                       *Plan is currently suspended
To participate, an employee must be 21 years of age and have one year of full-time service. The plan has a seven-year vesting schedule with the hospital contributing a minimum of 2% of the employee’s gross earnings in addition the hospital will match 50 cents on the dollar to what the employee contributes to their 457b up to 4% of gross earnings.  
*Part-time employees will be eligible for pension under the following conditions:
a)    Previous qualifying year (minimum of 1,000 hour worked);
b)    Worked a minimum of 1,000 hours in the current year.

Voluntary program of pre-tax savings through payroll deduction for retirement.

Section 125 “Cafeteria” Plan
Voluntary benefit that complements the health and dental insurance by allowing an employee to pay their insurance premiums with pre-tax dollars. It also allows the employee to set aside pre-tax dollars in an account to pay for non-reimbursed medical expenses and dependent care expenses. Enrollment in November of each year.

Group Term Life Insurance
To participate, an employee must have one year of continuous full-time service. UABMW will provide an amount equal to ½ of the employee’s annual salary, raised to the next highest thousand with a minimum amount of $10,000, up to $300,000. Dependant coverage is available also.
*Part-time employees will receive $10,000 in coverage if they work a minimum of 1,560 hours during the calendar year. Coverage will be for one calendar year and reviewed at year-end for possible continuation.

Voluntary Term Life with Critical Illness Insurance
Voluntary Family Protection Plan developed to provide the employee with life insurance protection and an instant emergency fund if an unexpected critical illness occurs. The employee will have the option of purchasing up to an additional $150,000 in coverage for you and your spouse and up to $20,000 for children.

Long Term Disability Insurance
To participate, an employee must have one year of continuous full-time service. LTD insurance provides for employee income replacement equal to 60% with a maximum benefit of $6,000 per month in the case of a long term disability greater than 90 days in duration.

Short Term Disability Insurance
To participate, an employee must have one year of continuous full-time service. STD insurance provides for employee income replacement equal to 60% of his or her salary with a maximum benefit of $1,000 per week up to 9 weeks in duration.
